Candidates should submit:
- A curriculum vitae highlighting education, professional
experience, professional publications and presentations,
public policy and legislative experience and committee
and advisory group appointments;
- A 500-word statement describing the applicant's
interest in the fellowship and what the applicant
hopes to learn from the experience;
- Two letters of reference from ADA member dentists.
